1. The number on the card
2. The Expiration Date of the card
3. The dollar amount
4. Your shipping address
5. If a P.O. Box, freight will be by USPS

An order isn’t placed until you’ve made payment. The waiting period for us to ship an amp is getting long at 30-60 days. The que is first paid, first served.
